Medical Boarding Information & Consent





Thank you for trusting us to take care of your furry family member for medically supervised boarding.
Rest assured that we will treat him/her like one of our own and make every effort to return your pet(s) to you safe, healthy and happy!
To provide a safe environment for all with minimal risk of infectious disease and parasites, we require that all of our guests be current on core vaccinations and be parasite free.

Vaccination:

Proof of current vaccination status is required for all pets. AAHA core vaccinations for dogs are DHPP (distemper, hepatitis, parvo, and parainfluenza), Rabies, and Bordetella; for cats, FVRCP (feline viral rhinotracheitis, calici, and panleukopenia) and Rabies. The 3 year Rabies vaccine is now accepted in Alabama. Bordetella must be given yearly. Extended protocols for DHPP and FVRCP are acceptable, but must be approved by a doctor.

Parasite Prevention:

Flees, ticks, heartworms, and intestinal parasites are not welcome here at 280 AMC. To keep your pet safe, proof of screening for parasites and use of prevention medication is required for our guests. If a pet is suffering from parasites, treatment will be provided at owner’s expense.

Facilities and Entertainment:

All guests are housed indoors in climate controlled areas with comfy Kuranda beds in our runs. Outside exercise areas are utilized and all of our guests get “TLC” at no extra charge. Dogs will enjoy a minimum of three outside walks per day.

Diet & Belongings:

High quality Purina Pro Plan Veterinary EN diet is fed twice daily to our guests. Special diets will be fed at no extra charge but must be provided by the owner. Toys, blankets, and goodies from home are welcome but are left at your own risk. For safety reasons, collars and leashes are not allowed (but we can keep them for you). Tear away ID collars are provided.

Medications:

Any required medications or treatments will be administered by a medical staff member.

Sunday Pick-up:

For your convenience, Sunday evening pick-up is available at 5pm. Prepayment is required as the practice is not open at that time. Arrangements must be made at check-in. No pets are admitted at this time.



Consent: I consent and authorize 280AMC to receive and board my pet(s). I understand that the doctors and staff will use all reasonable precautions for the safekeeping of my pet(s). I authorize the doctors to provide emergency care or treat minor concerns that may arise. I agree not to hold the hospital or its staff responsible for sickness, injury, or unforeseen events that may occur during my pet's stay.
